What this policy covers

This policy covers this website only. It does not cover the patient portal, the video-visit platform, or the care you receive in the clinic. Each of those is governed by its own privacy policy or notice.

Health information you share with us as a patient is protected under federal law (HIPAA) and our Notice of Privacy Practices, which is available at the front desk and on request. Nothing in this policy changes those protections.

Information we collect

Information you give us. When you use the contact form, we receive the name, email address, subject, and message you type in. That is the only form on this site. We do not collect payment information through this website. We delete contact-form messages stored on this website after 90 days, and we do not store your IP address with them.

Information collected automatically. Like most websites, ours records standard log data when you visit: your IP address, browser and device type, the pages you view, and the page that referred you. We use Google Analytics to understand how the site is used in aggregate. The embedded Google Map on the Contact page may also set cookies from Google. The contact form uses Cloudflare Turnstile to stop spam, which sends technical details about your browser and device to Cloudflare.

How we use it

We use what you give us to respond to your message, to return your call, and to answer your question. We use the automatically collected data to keep the site working, to keep it secure, and to see which pages are useful so we can improve them.

We do not sell personal information. Our website analytics may be linked to Google Ads, which lets Google use information about your visits to this website to measure how our ads perform and to show our ads to you on other websites.

Please do not send medical details through the contact form

The contact form is for general questions and scheduling. It is not an encrypted medical record, so please do not include symptoms, medications, test results, or other details about your health. For anything about your care, call us at (417) 332-3639 or send a message through the patient portal, which is built for that purpose.

Cookies and analytics

Cookies are small files a website stores in your browser. We use them for Google Analytics and for the embedded map. Because our analytics may be linked to Google Ads, Google may also use its cookies to show our ads to you on other websites. You can block or delete cookies in your browser settings, and Google offers a browser add-on that opts you out of Google Analytics. The site works without cookies.

Who we share it with

We share information only with the service providers that run this site for us: the company that hosts the website, the email service that delivers contact-form messages to our office, Cloudflare for the contact form's spam check, and Google for analytics. They may use it only to provide those services, except that Google may also use information about your visits to measure and show our ads, as described above. We will also share information when the law requires it. We do not sell it to anyone.

Links to other sites

This site links to the patient portal, the video-visit platform, Google Maps, Facebook, and Instagram. Once you leave trilakesfamilycare.com, their privacy policies apply, not ours. Online appointment booking is provided by Tebra: the booking window that opens from our Book buttons is run by Tebra, even though it appears over our pages, and what you enter in it is handled under Tebra's privacy policy, not this one.

Children

This website is for adults. We do not knowingly collect personal information from children under 13 through the site. If you believe a child has sent us information through the contact form, call us and we will delete it.

Security

We take reasonable steps to protect the information this site collects, including serving the site over an encrypted connection. No transmission over the internet is completely secure, which is one more reason to keep health details out of the contact form.

Your choices

You can use the site without the contact form; every page lists our phone number. You can ask us to delete a contact-form submission by calling the office. You can block cookies in your browser, and you can opt out of Google Analytics with Google's add-on. You can turn off personalized ads from Google in Google's ad settings (opens in a new tab).

Changes to this policy

If we change this policy, we will post the new version here with a new effective date. Using the site after that date means the updated policy applies.
